ELEMENTAL PRAISE £5 MILLION INJECTION INTO SOCIAL PRESCRIBING

This Month, we’re welcoming the news that the National Academy for Social Prescribing (NASP) has been awarded £5 million in funding to support people to stay connected and maintain their health and well-being following the Covid 19 Pandemic. The funding will connect  people to local community initiatives in their area to improve their mental health and well being in response to the aftermath of Covid-19.

CASE STUDY

Why Hydebank wood college partnered with Elemental to help lay the foundation for integrated care from all involved agencies and continue social prescribing through the gate and beyond.

 Located in the rural area of Belfast, Hydebank wood college is a category C Prison with Males (Age 18-21+) and females aged (Aged 18+) In 2019 the public Health agency funded a social prescribing pilot study that involved a multi- dispiclinary team within the college. This team would support a group of female patients to better connect into programmes and services that contributed towards improved Health and Wellbeing within the prison environment.

HOW SOUTH DUBLIN COUNTY PARTNERSHIP ENHANCED THEIR SOCIAL PRESCRIBING PROGRAMME VIA ELEMENTALS DIGITAL PLATFORM

In 2018 as part of their Health and Welling strategy, SDCP Launched a Partnership with Elemental software to develop a pilot social prescribing project, funded by Healthy Ireland and the social inclusion and activation programme, which enabled GPs within two neighbourhoods ro make instant referrals into its services.
