The Department has two divisions; San Francisco Health Network— the top-rated clinics, hospitals and programs—and Population Health, which offers public health services, disease prevention, and infection control.
The work is focused specifically on the San Francisco Health Network side of the department. Specifically, this new member is VP of Rehabilitation Services and the Executive Administrator for the Home Health Agency.
Their goal is to provide equitable, safe, timely, efficient, effective, and patient-centered care. As a healthcare system which provides Acute Care, Rehabilitation Services, Skilled Nursing, and home health, the Department of Public Health strives to make the process through the continuum of care is as seamless as possible.
Th Departmenet of Public Health San Francisco is looking forward to connecting with other leaders across the globe to understand how leaders have navigated barriers in the continuum of care, connect to see how other leaders are utilizing the home to provide care that may have previously been provided only in a hospital setting. Essentially, they want to learn from all of the great leaders across the globe and connect with other change makers in their respective industries.
