Harmony Health’s platform amplifies the digital voice of the invisible trusted ambassadors of the community. Care givers, FQHC clinics, Community-based Organizations, Faith leaders, and Local influencers that have created deep, trusted relationships with underserved communities, a powerful asset not easily duplicated.
They are building the infrastructure to digitize trust in communities and leverage the invisible trust fabric beyond state and national boundaries. The platform is used by Invisible Ambassadors to communicate culturally relevant content in their own voice and language using SMS and other channels. They are able to build localized communities, rapidly add and onboard members and work with multiple partners (CBO’s, providers, health plans) to engage. The platform is very user friendly and easy to deploy.
Harmony Health has partnered with CCHI (California Coverage & Health Initiatives) in serving members and underserved communities throughout California. Key community stakeholders, local non-profits, and faith-based organizations have used Harmony Health to leverage their trusted networks, connect digitally with their communities, and provide needed resources. This initiative covered 800k+ community member across 21 CBO (Community Based Organizations) partners and 200+Trusted Ambassadors. Using the power of trusted networks, the initiative generated 350K CTAs and 15k enrollments and social service referrals among lower income older adults in California.
