Strata Health UK Improves NHS Borders’ Patient Discharge Workflow full integration to Scottish Care Information (SCI) Store

NHS Borders clinicians can now access real-time patient data and information more efficiently, improving the quality and outcomes of referrals to downstream services, thanks to a newly deployed integrated interface between Strata Health and SCI Store.

Using a single set of credentials, clinicians can log in to the Strata Health platform, auto-populate forms with patient information stored within SCI Store, and process and send referrals to downstream services in just a few clicks.

“The SCI Store integration provides our clinicians and social workers with seamless access to a central source of demographic data for patients and clients,” said Suzan Bell, Digital Change Manager (H&SC and CYP), Business Change & Improvement at Scottish Borders Council. “It means our front line staff are spending less time re-keying information from paper records or other systems, which reduces the risk of errors when this data is copied. Ultimately, this means our front line staff spend less time on administrative tasks and more time face-to-face with their patients and clients.”

With Single Sign-On (SSO) capability and real-time data exchange, the integrated interface achieves greater interoperability amongst NHS Borders’ digital health systems. The seamless flow of patient information ensures data integrity and streamlines referrals, simplifying workflows for clinicians, improving transitions of care, and ensuring patient’s experience a smooth, effective and safe care journey.

“The integration with SCI Store allows Strata Health to provide an out of the box interface to the NSS information repository,” said Wasim Shabir, Director of Interoperability at Strata Health UK. “Its power enables healthcare professionals to seamlessly access and interrogate the repository and include data sets of interest within their clinical pathways at a click of a button, saving front line staff valuable time.”
