Strata Health is supporting the Scottish Government Mental Health Strategy and working with partners to develop a single-point-of-access for use in social prescribing. Social prescribing enables health and social care providers the ability to refer patients to non-clinical services that holistically support their health and well-being. This helps deliver more efficient care to patients, at the right time.
Strata Health is pleased to announce a new partnership with NHS Western Isles and mPower to utilise Strata technology in the development of a proof-of-concept for a Single Point of Access (SPoA) that can be used in social prescribing for mental health and wellbeing.
The Scottish Government Mental Health Strategy and its Good Mental Health for All initiative place significant emphasis on the potential of social prescribing to improve population health and outcomes. Social prescribing enables Community Navigators, Link Workers, GPs, nurses, social care, housing, ambulance services, families and other health and social care providers to refer patients to non-clinical services that holistically support their health and well-being. Social prescribing includes a range of third sector voluntary and digital resources and services, such as digital Cognitive Behavioural Therapy, companionship, arts therapy, gardening, healthy eating advice and group sports.
Providing a single point of access for mental health and social prescribing to both third sector organisations and digital social prescribing programmes enables a health-promoting and preventative approach which will improve responses to common health problems and support integration of community, in-patient and crisis mental health.
The team at Strata Health is incredibly excited to be working on this critical proof-of-concept partnership, which has enormous potential to improve social prescribing processes and outcomes. We look forward to continuing to work with an innovative health, social and community team to advance access to services for local people in the Western Isles.
