Nurses Use Strata PathWays to Receive Care at Home Services for the Terminally Ill

End of life care for terminally ill patients and their families is further enhanced through the use of Strata PathWays. This integration will improve the quality and consistency of referrals and care requests. This implementation will improve the quality of patient care by providing the optimal use of at-home services, at the time they are required, thus allowing the person and their family to remain at home or in their preferred location rather than emergency hospital admission.

Strata Health is pleased to announce the integration of Strata Pathways with district nursing in the Scottish Borders for services to support palliative care. End of life care for terminally ill patients and their families is further enhanced through the use of Strata PathWays which enables district nurses to make referrals and access and secure packages from their local Care at Home services.

The project benefits for providers and patients include:

  • Improved quality and consistency of referrals and care requests
  • Proper care sources so patients receive the right support for their unique care needs
  • Automating administrative tasks for supporting clinicians
  • Integrated working across health and social care

District Nurses play a central role in the management and coordination of care for palliative and terminally ill patients. This implementation will improve the quality of patient care by providing the optimal use of at-home services, at the time they are required, thus allowing the person and their family to remain at home or in their preferred location rather than emergency hospital admission.
