The CRANE Project (Comprehensive Treatment of Chronic Patients in Rural Areas) is an Innovative Public Procurement Project funded by the European Union that aims to build a system to increase person-centred care and evolve towards a model in which more than 80% of chronic patients move to self-care. The technological solution proposed by CRANE will be validated in 2025 in two locations in Extremadura-Spain (Jarandilla de la Vera and Villanueva de la Vera) and in several municipalities in Norway and Sweden.
Currently, 3 European companies are designing a prototype of the possible technological solution and each of them carried out, during the month of November, co-creation sessions in which patients with chronic diseases (Diabetes, Cardiovascular Diseases and Chronic Obstructive Pulmonary Disease) from Jarandilla de la Vera and Villanueva de la Vera participated, health professionals from the SES (Extremadura Healthcare Service) and other stakeholders from FundeSalud (Foundation for Training and Research of Health Professionals in Extremadura), SEPAD (Extremadura Service for the Promotion of Autonomy and Care for Dependency) and the Ministry of Health and Social Services of the Regional Government of Extremadura.
The first of these sessions, took place last 16th of November at the Espacio Joven in Jarandilla de la Vera, and 14 patients, 5 healthcare professionals, 3 family carers of patients with chronic diseases and 2 interested agents attended the event. At this meeting, one of the 3 companies competing to design the best solution for patients presented a mock-up of the App they are developing so that attendees could assess how this technological solution could benefit them.
