Implementing Strata PathWays allows Blackpool Teaching Hospitals to improve the patient flow experience and increase availability of hospital beds. This improves discharge delays, improves referral quality and accuracy, and in turn improves patient outcomes. This integrated solution allows hospital nursing staff to make a single, simple referral to the SPOD where the patient is reviewed, triaged and signposted into the most appropriate downstream out-of-hospital service

Pilot Project to Introduce Single Point of Discharge Now Ready to Expand to All Wards Trust-Wide.

(June 2021) The Strata Health UK team is working with Blackpool Hospitals to expand Single Point of Discharge Services (SPOD) to a trust-wide implementation.

Originating as a pilot-project in four wards, this integrated solution allows hospital nursing staff to make a single, simple referral to the SPOD where the patient is reviewed, triaged and signposted into the most appropriate downstream out-of-hospital service.

Single Point of Discharge enables Social Care and NHS colleagues to work in partnership to streamline referral management processes, reduce delays associated with multiple handovers and support a more efficient service for patients.

Prior to implementing Strata Pathways™ software, the SPOD team faced a referral process that required multiple phone calls and follow-ups, resulting in delays and bed-blocking. Transitioning to Strata Pathways™ software allowed for reporting in real-time, offering a management tool to provide insight to inform strategy and to see trends and act before a problem escalates. Based on the pilot’s exceptional success, the decision was made to implement a full trust-wide rollout as quickly as possible.

The program rollout will be supported by a monumental 5-week training exercise coordinated by over 50 Matrons and Ward Managers. Blackpool staff’s significant efforts will help ensure the seamless integration of Strata PathWays™ software, delivering proven results trust-wide.
