Better, in partnership with OneLondon, has won the Health Tech Digital Award 2023 in the category Digital Transformation Project of the Year for Universal Care Planning: Deploying OneLondon’s urgent care planning solution across 5 ICSs in only 7 months.

Through transformative digital solutions and the outstanding collaboration between London providers, the Universal Care Plan enables every Londoner to have their care, health status, and support wishes digitally accessible with health and care professionals across London. The overall vision of London is to become the healthiest city globally, and with the Better Digital Health Platform now deployed, the rapid future development of needed digital services to support patient pathways is enabled. 

The solution was delivered in just seven months. The first use case for the dynamic integrated care planning platform – end-of-life Universal Care Plan (UCP) – is supporting a population of approximately 10m people, across 5 Integrated Care Systems, over 40 NHS trusts, and 1,400 general practices. Up to now, 53,316 living Londoners have a personalised care plan, and 78% of Londoners who had a UCP and died did so at their preferred place of death.

The UCP platform enables a truly person-centred approach expanding this year with frailty, dementia, and wider personalised care and support and beyond. The award is a recognition of the great collective work, combined with using an open data and low-code approach to building technology, and we are excited to see where the future will take us,” said Matt Cox, Managing director of Better UK & Ireland.

OneLondon, together with Better, is now looking at how to transform this universal care plan for patients on different patient pathways and there is a development plan in place to extend to other care planning use cases. This includes, but is not limited to, supporting people with personalised health and care plans, sickle cell disease, dementia, frailty, children and young people, asthma, mental health needs, learning disabilities, autism and mental health.
