The Elemental Open Lounge – Proactive Social Prescribing

Join us for our monthly Elemental Open Lounge to find out more about our award-winning digital social prescribing platform and how it is being used to enhance social prescribing for hundreds of organisations in health, housing, education, local government, employment and social care throughout the UK and Ireland.

Each month we choose a theme to focus on, highlighting the importance of the theme and how Elemental helps. This month’s theme is Proactive Social Prescribing.

From September this year, the Network Contract DES specification will require Primary Care Networks to provide a proactive social prescribing programme to an identified cohort with unmet needs. Elemental’s suite of reports, along with Population Health Management data, can help to identify this cohort and then track progress against referral targets and outcome measures.
