Sustainability and effectiveness of healthcare systems due to an ageing population and recent COVID-19 pandemics are the main catalysts of change and growing demand for coordinated and integrated health and social care services, with citizens at the centre of this change. Integrated care has been implemented in various forms and at different pace in Europe and worldwide. It is therefore vital to capture this breadth of experience and knowledge and effectively share the learning to build the capacity of health and social care authorities to adopt and scale up integrated care solution which are the foundation and aspiration of ECHAlliance Integrated Care Thematic Ecosystem.
