Integrated service: organizational healthcare models in the framework of chronic diseases Conference

Alexia Zurkuhlen, an ECHAlliance International project manager anHealthRegion CologneBonn presented at the conference in the session “The integrated care services addressed to older people with chronic condition”.

Alexia presented a number of integrated care approaches, most of them based on digital health, taking place in EU regions having filed a “commitment” to the EIP on AHA B3 action group “integrated care”.

After clustering those commitments according to regions, approaches and main focis, Alexia went into more detail for a selection of those commitments, discussing the challenges they were facing. Finally Alexia presented the latest endeavour of a rural region around Cologne that submitted a proposal for an integrated care approach for chronically ill 65+ adults who suffer from the lack of health and care professionals in the regions and the almost inexistent communication between both sectors in general.
